After Hours Urgent Calls

For a true medical emergency please call 911. For other urgent questions or issues, our answering service will contact the physician on call when our phones are unavailable.

Medication Refills

It is our policy to see patients at least once yearly for ongoing prescriptions to be refilled. Depending on your condition and/or medication, it may be necessary for you to be seen more frequently than once a year to have an ongoing prescription refilled. For medication refills, please contact your pharmacy to have a refill request faxed to our office directly.

Referrals

Your insurance company may require that you first have a referral from your primary physician in order to be seen by our practice. Please be sure that this is done PRIOR to your visit. If this is not done, we are required to have you sign a waiver (see WAIVER below). We apologize for any inconvenience that this may cause.

Waivers

A waiver is a statement which says that you understand the services in our office may not be covered by your insurance and that you would be responsible for the charges. This may be necessary in several circumstances: if a referral is required by your insurance company and none has been made; if you did not bring your insurance card; for cosmetic and other procedures not covered by your insurance.

Payment Policy

Our office participates with most major insurers. Please bring your insurance card with you at the time of your visit.

All Co-pays will be due at the time of service.  If you have a deductible health plan, a deposit of $50.00 will be due at the time of service. 

We accept the following: cash, check, and most major credit cards. Our office is willing to assist you with any problems that may arise and can work out a payment plan with you if necessary. Please ask to speak with someone in Billing to assist you with any further questions.

No-Show Policy

A fee of $25.00 will be charged if an appointment is no-showed.  If there are multiple no-showed appointments it could result in discharge from the practice. 

Pathology

It may be necessary for the physician to send a sample of your skin for microscopic analysis. We default samples to Muhlbauer Dermatopathology Labs in Pittsford. The skilled pathologists in this laboratory are medical doctors who are board-certified in the preparation, examination and interpretation of skin biopsies. Their fees are entirely separate from those of our practice, and Muhlbauer will bill insurance companies separately.

We acknowledge that some insurance carriers require skin samples to be routed to a separate microscopic lab, please be aware of this information prior to your visit.
